This is a hayden super valve or direct connect wall hose valve for most brands of central vacuums. This wall valve has both low voltage hook ups and 110 right in the valve. It comes with the face plate and the mounting bracket.

This inlet has a square door with screws that are visible with the door closed. The reversible flange allows the inlet to be installed on either side of a stud.
